Payment, billing, and insurance options are available for care

Cleveland Chiropractic and Integrative Health Center, LLC is in-network with Medical Mutual, Mutual Health Services*, Anthem Blue Cross Blue Shield*, United Healthcare, UMR, United Healthcare Community Plan, Medicare, Multiplan, and a few smaller insurance companies.  

As a reminder, your insurance policy is an agreement between you and your insurer, not your insurer and this clinic.  Like all types of care, coverage for chiropractic and acupuncture services vary from insurer to insurer and plan to plan.  Most insurance policies require the beneficiary to pay co-insurance, co-payment, and/or deductible.  Our clinic will call your insurer to verify your benefits; however, we are not responsible for your insurer’s final payment and benefit determinations. 

For your convenience, we are happy to send insurance claims to your insurance carrier. Payment is expected at the time of service if your plan has a copay, coinsurance, or if you have not yet met your deductible.  If you have questions about the network status or your policy, please call the office (216) 952-3830 or your insurance carrier. Get to Know Your Insurance Download

* Please note: our office is not in-network with Cleveland Clinic Employee Health Plan (EPH) or University Hospitals Employee Health Plan.

** Please note: at this time, we are not able to bill your insurance carrier for massage therapy services.  Visit the massage page for massage therapy services and prices here. Chrissy (our physical therapist) is an in network provider with Anthem BCBS and Medical Mutual. Private pay prices are listed in our online scheduler.
